INITIAL VISIT

TESTING

We will perform a thorough physical exam. Occasionally, urodynamic testing is performed to evaluate bladder storage and emptying. Additionally, cystoscopy to evaluate the bladder and vaginoscopy to evaluate the vagina may be needed. Thse procedures are painless and done with anesthesia in our surgery centers.

MINOR PROCEDURE

Surgery is performed robotically, laparoscopically, or vaginally. Procedures typically take 1 to 2 hours and you are sent home the same day. Surgery is covered by all major insurances.

TYPES OF PROLAPSE

1. Cystocele – This is prolapse of the bladder and often requires manually pushing the bladder back in the vagina to urinate or empty completely. Symptoms include pelvic pressure, frequency, urgency, and incomplete emptying.

2. Rectocele – This is prolapse of the rectum, most often associated with constipation. This bulge will occur during straining especialyl with bowel movements.

3. EnteroceleThis is portrusion of bowel contents through the vaginal wall.

GENERAL TREATMENT OVERVIEW

1. Lifestyle Changes – Lifestyle changes include a plant based diet, water (avoid coffee, tea, and soda), stress reduction (yoga and meditation), sleep improvement (treat sleep apnea and monitor REM sleep), exercise modification and weight loss.

2. Medications – Medications such as estrogen or progesterone are sometimes used to help increase the strength of tissues prior to surgery.

3. SurgerySurgery is generally performed laparoscopically, robotically, or vaginally. Generally open surgery is not needed for over 95% of patients.

REPAIR APPROACHES

1. Vaginal (Anterior Colporrhaphy, Sacrospinous Ligament Fixation) – A vaginal approach for prolapse repair is done without the use of mesh. This is the least invasive way to repair prolapse.

2. Laparoscopic Sacrocolpopexy – Small holes are made in the abdomen and instruments are used to fix the vagina to the sacrum bone with mesh. You are sent home the same day. This is highly effective with great outcomes.

3. Robotic Sacrocolpopexy – Small holes are made in the abdomen and robotic instruments are used to fix the vagina to the sacrum bone with mesh. You are sent home the same day. This is highly effective with great outcomes. The robot is controlled by our expert surgeons.

USE OF MESH

One of the most effective ways to treat severe prolapse is the use of mesh placed through a robotic or laparoscopic approach. This is very safe, effective, and has incredibly high cure rates over 10 years time. The American Urologic Association and Society for Female Urology support the use of mesh.

NON MESH REPAIRS

We can perform non mesh repairs when needed. These repairs are done by our expert surgeons in our surgery center generally through a vaginal approach. This means there is no incision in the abdomen.
